The tensor fascia lata myocutaneous flap has been used for the reconstruction of abdominal wall defect, perineal defect, pressure sore coverage of the greater trochanter and ischium, and this flap is also used as a free flap.
The use of the tensor fascia lata myocutaneous flap has increased due to many advantages of this flap, that is, high reliablility, versatility, safety, large skin territory and closing the donor site primarily. But, we have encountered, in a significant number of cases, the necrosis of the superior portion of the donor area after closing the donor site primarily, because of the excessive undermining for advancement and the distance from the blood supply.
Authors carried out the bilobed tensor fascia lata myocutaneous flap operation for solving the excessive tension developed in the proximal portion of the donor site in two large greater trochanter pressure sore patients with good result.
